Once you escape, Reimu will arrive to save you. If you haven't completed Kaguya's request, Reimu will kill you. But if you have, she will instead take you back to the shrine and then back to the Outside.
There's also a secret ending where you instead escape through a gap. It involves a patient named "Merry".
